Step 1: Prepare Your Medical Records
Our team members will ask you or your local physicians for all medical records relating to your condition.
In order to diagnose and treat unusual or persistent symptoms, as well as check your overall health, Doctor Limas may order one or more laboratory tests. These important tests can help you and your doctor manage and improve your health.
Your records must be in English or Spanish. If your Doctor or Laboratory cannot provide English or Spanish copies of your medical records, you must have them translated by a certified medical translator. (Third-party, fee-based translation services are available upon request.)
